Illness can often cause crisis in patients or their families. The medical social worker presents resources available and helps families plan for ongoing care, helps them meet emotional and social needs and gives them support. He explains health care policies and helps patients to cope with all the problems which hospitalisation can cause. He involves other health care professionals as needed and makes available all the services which the patient and his family are entitled to. Sometimes he will continue care after the patient is discharged, or make contact with community social workers to ensure continued care. Most medical social workers work in hospitals, hospices or nursing homes.
